Walk in Frailty Hub

Starting from 3rd August 2023

• Do you have concerns about a relative who is 65 years and over?
• Are they having falls or struggling with day-to-day tasks or becoming frail?
• Are you concerned about their memory?

Have a chat with them and then come along to our weekly walk-in frailty hub based at Springhead Surgery on Thursdays from 10am to 11am where you can speak with a care co-ordinator who can signpost you to the local support networks who aim to provide support and maintain the independence of our aging population.

You will be surprised at the support that can be offered.
